We are looking for a hard-working, dependable, and positive person to join our amazing team at The Retreat and in Community Engagement. The Retreat is a day program for adults with intellectual and developmental disabilities. This position will provide services to individuals both in the day program and in community engagement.
Direct Support Professional Job Description
Position Title: Direct Support Professional (DSP)
Reports To: Program Assistant
Objective of Position:
To carry out services and supports as outlined in the individuals Person Centered Individualized Service Plan.
Provide direct care and services to individuals with developmental disabilities with/without physical disabilities and with cooccurring mental illness.
Qualifications of Position:
Minimum of a High School Diploma or GED.
Must be able to pass a Criminal History and Child Protective registry checks.
Must have a valid Driver’s license with a satisfactory DMV score (positive points balance, no DUIs or reckless driving within a predetermined amount of time).
Must obtain a passing Tuberculosis screening.
Duties of position:
Knowledge of My Life My Community services.
Read, implement, and follow the service plan for the individual receiving supports.
Developing and maintaining professional friendships with person of all abilities including handling social encounters
Supporting the individual in replacing challenging behaviors with positive, accepted behavior;
Monitoring the individual's health and physical condition and providing supports with medication and other medical needs;
Providing safety supports with transportation to and from sites and community resources;
Providing general and safety supports, as needed to ensure the individual's health and safety;
Provide and arrange for the provision of appropriate medical care as directed by the supervisor;
Complete and submit all required documentation in a timely manner and in the format required by Community Connection Support Services. Required documentation includes but is not limited to completion of daily progress notes, daily logs, MAR, medical appointment, incidents, contact notes, etc;
Recognize, respond, document, and report emergency medical, behavioral, psychiatric conditions, crisis or other unusual conditions;
Maintain the privacy, security and confidentiality of the individual’s information as required by the Health Insurance Portability and Accountability Act;
Follow all Community Connection Support Services policies and procedures.
Demonstrate a commitment to ethical service provision using the Code of Ethics for Direct Support Professionals;
All Direct Support Professionals are “mandated reporters” and must report all suspected abuse and neglect upon discovery to the Adult Protective Services and the Director of Operations at Community Connection Support Services;
Attend and participate in all required trainings and staff meetings;
All other duties as assigned by supervisor.
KNOWLEDGE, SKILLS, AND ABILITIES OF THE DIRECT SUPPORT PROFESSIONAL:
Knowledge of: Principles and philosophy of developmental disability services; concepts of positive behavioral supports; specialized and general knowledge of the implementation of individualized training programs needed to implement individualized service plans; documentation and data collection systems;
Skills of: Oral and written communication; interpersonal skills particularly in terms of building trust and rapport with persons who have special needs; observing and documenting behavior; responding to the special needs of people with disabilities; implementation of training programs for individuals with special needs. Utilizing resources to maximize the individual’s service experience.
Ability to: Read and understand the individual’s service plan, observe and document observations, implement services; Sit, stand, stoop, see, hear, smell, use hands to grasp, arms to push, pull, lift, use legs to walk and run. Must be able to lift/move individuals as needed; Appropriately implement professional confidentiality practices; Demonstrate a commitment to quality, community-based services for people with disabilities, adhere to policy and procedures set forth by Community Connection Support Services. Recognize and adhere to the Final Rule regulation 42 CFR Part 441 regarding Home and Community Based Services.
